Job Summary
Saint Anne is a member of the Benedictine Health System, a Catholic non-profit sub-acute and long-term care skilled nursing facility. Saint Anne's has 109 private skilled nursing beds, 10 being in the Transitional Care Unit, and a 75-unit Assisted Living all on one campus. The therapy department is in-house, not contracted, and has state-of-the-art technology in ceiling tracked gait training systems. The resident caseload is adult and older adult populations; inpatient sub-acute, long-term care and outpatients. This position is a full time position (80 hours per pay period) Monday through Friday with occasional weekends as needed.

QUALIFICATIONS REQUIRED
  • Current state licensure as a Physical Therpay Assistant, which must be in good standing
  • Excellent interpersonal skills
  • Basic Computer skills(Word, Excel, Outlook)

  • One year previous experience in long term care

    ESSENTIAL FUNCTIONS OF THE JOB, DEPARTMENTAL EXPECTATIONS
  • Perform specific assessment components of treatment and report findings to physical therapist (i.e. Tinetti Balance and Gait Scale).
  • Provides treatments to meet individual's needs (physical, cultural, and psychosocial) per plan of care. Respond promptly to acute changes in resident's physiological state and report to physical therapist; modify or adjust treatment plan within limits of the plan of care.
  • Communicate resident progress and response to treatment effectively with supervising therapist and other interdisciplinary staff members.
  • Resident/family education: Plans for and provides resident, family and caregiver education, identifies learning needs, clearly communicates information, and documents response to education provided.
  • Document and complete billing according to current standards and policies.
  • Discharge planning: Plans for resident discharge, anticipates long term and short term resident needs, and collaborate with interdisciplinary team and make recommendations for discharge to physical therapist.
  • Professional Growth: Attends meetings, workshops, seminars and participates in committees as requested/required by facility.
